Community Reinvestment Policy

San Juan Regional Medical Center is committed to supporting the community in a variety of ways which includes a Community Reinvestment Fund. All applications for community reinvestment funding must meet criteria/guidelines as set forth below. The Community Reinvestment Fund Program is not for fundraisers, sponsorships or special events. Requests for those categories may be made through Community Relations where other specific criteria must be met.

Priorities:

Funding priorities for the Community Reinvestment Fund will focus on the most recent Community Health Needs Assessment (see link on right side of page) conducted in the spring of 2008 and the health needs identified in that report. Requests for funding should also further San Juan Regional Medical Center's Mission, Vision and Philosophy.

Types of Funding:

San Juan Regional Medical Center will fund projects and opportunities in the following ways:

  • Seed Money: This category is for a new project needing funding to get started. The funding will be available for three years with the amount being reduced by 50% each year. For example, if the original request is for $10,000, the maximum amount awarded for the second year would be $5,000; and $2,500 for the third year.
  • One Time Funding: This would be funding for one specific amount with no further funding needed.
  • Ongoing Funding: This would be for projects deemed appropriate for addressing a community health needs on a continual basis. Annual review of the project is mandatory.

Budget:

A budget must be submitted with the request. The maximum that will be funded is 40% of the total budget for seed money and one time funding projects.

Annual Reporting:

Applicants must submit an annual report on current and open projects including performance will be made in the first quarter of the fiscal year.

Proof of Not for Profit Status:

Applicants must submit a current IRS determination letter as proof of your organization's not for profit status.

All applications for funding must be in writing and address the following:

  • All requests must benefit the service area of the hospital;  San Juan County, NM (Farmington, Aztec, Bloomfield and surrounding area);
  • A description of both the organization and the project for which the funds are requested;
  • The community need being met and how the program will provide measurable improvement in the community with specific performance measures;
  • Other organizations supporting the project or organization;
  • How the program is consistent with the Mission, Vision and Philosophy of SJRMC;
  • Project/Program coordinator;
  • Whether volunteers will be used;
  • Whether consultants will be hired; if so, submit budget for consulting fees.
  • Post Funding Reporting. An executive summary must be submitted in writing 90 days post funding. Failure to submit a post funding report could impact future requests for funding. The summary must include the following information:
    1. What benefit did the funding provide;
    2. How many people were served;
    3. If applicable, how many volunteers were used to complete the project;
    4. How many paid personnel and/or consultants were used and what was the cost;
    5. Submit measurement to date;
    6. What problems arose during the first 60 days and how were they addressed;
    7. Any other information that is pertinent to the project that the organization would like to share;
